Summit State Bank provides charitable donations and event sponsorships based on our key areas of focus. Priority is given to those organizationsthat do business with us, conduct business in our service area and have a current 501 (c) status.

We do not provide funding for the following:

• Individuals
• Programs serving resident communities outside of our geographic footprint
• General operating requests
• Political organizations or advocacy
• Religious organizations
• Organizations that discriminate based on race, national or ethnic origin, religion, age, sex, sexual orientation, marital status, family status, veteran status, disability or other characteristics protected by law.

If your organization meets our Giving Criteria and you would like to request either a charitable donation or event sponsorship, please complete theappropriate request form and follow the instructions below.

All completed applications much be accompanied by:

• Scanned copy of documentation of your organization’s valid tax-exempt status, unless you are known to us, and we have your documentation on file (i.e., the IRS determination letter)
• Most recent IRS issued W-9 form (W-9 forms can be found on the IRS website)

All event sponsorship requests must be accompanied by:

• Scanned copy of event brochures or materials
• Detailed listing of sponsorship benefits and deadlines
• If advertising is part of the sponsorship, please provide specifications for file format, color vs. black & white, size and desired due date
